A Life Care Plan is a dynamic document based upon published standards of practice, comprehensive assessment, data analysis and research, which provides an organized concise plan for current and future needs with associated cost for individuals who have experienced catastrophic injury or have chronic health care needs.
Simply put, a Life Care Plan is a document prepared by a Life Care Planner, who examines the issues of the person’s illness or injury and determines what that person needs, now and for the remainder of their life. The goal is to bring them either back to as close to their pre-injury/pre-illness state as possible, or as in the case of a birth injury, as close to a “normal” existence as possible.
